Jequirity and Ciclesonide

Here is the interaction we found between these two, with the evidence behind it.

Minor

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Ciclesonide

Ciclesonide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Jequirity interacts with.

In vitro research shows that jequirity extract inhibits CYP3A4 enzymes. Theoretically, taking jequirity may inhibit the metabolism of CYP3A4 substrates.
Severity Minor Likelihood Unlikely Evidence D

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

First things first: jequirity seeds are genuinely poisonous (they contain abrin, a toxin close cousin to ricin), so this isn't something you should ever take as a supplement. Please don't use it at all. That matters far more than any interaction with your inhaler.

The interaction we have on file is theoretical and comes only from lab testing, not from people. In a dish, jequirity extract slowed down an enzyme called CYP3A4, which is the same enzyme that clears your ciclesonide. In theory, that could let a little more of the active drug hang around, which might mean slightly more steroid effect (think hoarseness, throat irritation, or oral thrush). The evidence here is weak and the risk is rated minor and unlikely. Either way, skip the jequirity, and chat with your pharmacist if you have questions.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Ciclesonide page

“Ciclesonide (Alvesco, Omnaris, Zetonna) is a corticosteroid for asthma and nasal allergies, and even though it is inhaled rather than swallowed, its supplement list warrants genuine care. The body clears it through the CYP3A4 enzyme, and 121 of the 166 ingredients on file nudge that pathway one way or the other.”
