CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Dofetilide
Dofetilide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Melatonin interacts with.
Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with melatonin and dofetilide (Tikosyn). Some lab studies suggest melatonin might slightly slow down CYP3A4, one of the enzymes that helps clear certain drugs. In theory, that could let a drug build up a bit. Dofetilide is not a prodrug, so if its levels did rise, you'd worry about too much effect rather than too little.
The reason I take this seriously is that dofetilide is a narrow-window heart rhythm drug, where even small increases can stretch the heart's QT interval and trigger dangerous rhythms. That said, this is only theoretical, animal and test-tube data, and rated minor. Please don't stop either one on your own. Just check with your pharmacist or cardiologist before adding melatonin.

Melatonin is a hormone your body makes naturally to help control your sleep-wake cycle, and the supplement form is widely used to help with sleep timing problems and jet lag. The evidence is strongest for certain circadian (body-clock) disorders and is more mixed for everyday insomnia. It is generally well tolerated for short-term use, but talk with your pharmacist or doctor before starting, especially for children or during pregnancy.

What Is Dofetilide Used For?
Brand name: Tikosyn
Dofetilide capsules are used in two main ways for people with atrial fibrillation (AF) or atrial flutter (AFl) — abnormal, often rapid heart rhythms. First, they can help convert these abnormal rhythms back to a normal heart rhythm. Second, once a normal rhythm has been restored, dofetilide can help maintain it and delay the time before the abnormal rhythm comes back.
Because dofetilide can cause dangerous heart rhythm problems of its own, it is reserved for patients whose atrial fibrillation or atrial flutter causes significant symptoms. It has not been shown to be effective for paroxysmal atrial fibrillation (short, self-terminating episodes). The brand Tikosyn carries the same two approved uses.
