Why this applies to Mavacamten

Mavacamten falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Melatonin interacts with.

Theoretically, melatonin might increase levels of drugs metabolized by CYP2C19. Also, other CYP2C19 substrates might decrease the metabolism of melatonin, increasing melatonin levels.
Melatonin is metabolized in the liver primarily by the CYP2C19 and CYP1A2 enzymes. Theoretically, combined administration of melatonin with certain drugs metabolized by the CYP2C19 enzyme may reduce the metabolism of these drugs, resulting in increased serum levels. Conversely, some drugs metabolized by CYP2C19 may inhibit the metabolism of melatonin, resulting in increased serum levels of melatonin. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the gist for Camzyos (mavacamten), which you take to ease the thickened heart muscle of hypertrophic cardiomyopathy. Your liver clears it partly through an enzyme called CYP2C19, and melatonin leans on that same enzyme. The theory is that melatonin could slow mavacamten's breakdown a little, letting more of it linger in your blood. Mavacamten is an active drug, not a prodrug, so higher levels could mean a stronger effect than intended, things like dizziness, shortness of breath, or your heart's pumping function dropping too low.

Honestly, this is theory based on lab pharmacology, not real patient reports, so I wouldn't panic. But mavacamten has a narrow safe range and requires regular echocardiograms, so let your cardiologist or pharmacist know before adding melatonin.

Here's the gist for Camzyos (mavacamten): your liver clears it largely through an enzyme called CYP3A4. Some lab research suggests certain batches of melatonin can mildly slow that enzyme down. If that happened in the body, mavacamten could clear a bit slower and its levels could creep up, which in theory means more of its effects, like a weaker heart squeeze that can cause shortness of breath, swelling, dizziness, or fatigue.

That said, this is theoretical and only comes from lab data, not real-world cases, so I wouldn't lose sleep over it. Still, mavacamten is a sensitive medication with regular echo monitoring, so it's smart to mention any melatonin use to your cardiologist or pharmacist before combining them.

What Is Mavacamten Used For?

Brand name: Camzyos

Camzyos (mavacamten) is approved for adults with symptomatic obstructive hypertrophic cardiomyopathy (HCM) — a condition where the heart muscle grows too thick and blocks blood flow out of the heart. Specifically, it is used in people whose symptoms fall into New York Heart Association (NYHA) class II or III, meaning they have noticeable limitations on physical activity.

The goal of treatment is to improve how much the patient can do physically and to reduce the symptoms that HCM causes, such as shortness of breath, chest pain, and fatigue.
