IMMUNOSUPPRESSANTS
Why this applies to Mirikizumab-mrkz
Mirikizumab-mrkz falls under IMMUNOSUPPRESSANTS — the group Melatonin interacts with.
Melatonin can stimulate immune function. Theoretically, melatonin might interfere with immunosuppressive therapy.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the quick version for anyone on Omvoh (mirikizumab), which is a biologic antibody used to calm the overactive immune response in ulcerative colitis. Because it works by dialing your immune system down, the concern is that melatonin may nudge in the opposite direction. Melatonin can mildly rev up immune activity, so in theory it could work against what Omvoh is trying to do and make your gut symptoms harder to control.
Honestly, this is a theoretical worry, not something proven in people taking this exact drug. The evidence is thin. Occasional short-term melatonin for sleep or jet lag is likely a small concern, but I wouldn't start a regular high-dose routine without first checking with your gastroenterologist or pharmacist.

From our pharmacist’s overview on the Mirikizumab-mrkz page
“Mirikizumab (Omvoh) treats inflammatory bowel disease by dialing down part of the immune system, and its interaction list deserves genuine wariness, most of all around the liver. About Melatonin
What is Melatonin?
- Trouble falling asleep (insomnia)
- Jet lag
- Delayed sleep phase syndrome
- Non-24-hour sleep-wake disorder
- Shift-work sleep problems
- Sleep problems in some children with conditions like autism or ADHD
Melatonin is a hormone your body makes naturally to help control your sleep-wake cycle, and the supplement form is widely used to help with sleep timing problems and jet lag. The evidence is strongest for certain circadian (body-clock) disorders and is more mixed for everyday insomnia. It is generally well tolerated for short-term use, but talk with your pharmacist or doctor before starting, especially for children or during pregnancy.
