Here's the idea behind this combo. Temsirolimus (Torisel) is a cancer medicine that also quiets down part of your immune system as it works. Melatonin, on the other hand, may gently nudge your immune system to be a bit more active. So in theory, taking melatonin could work against what Temsirolimus is trying to do. This isn't about melatonin building up the drug or causing more side effects, it's more of a tug-of-war on the immune system.

I want to be honest with you though: this concern is mostly theoretical and based on limited, lower-quality research. We don't have strong proof it actually weakens cancer treatment in real life. Since this is a serious medication, please don't start or stop melatonin on your own. Run it by your oncologist or pharmacist first so they can weigh in for your situation.

Temsirolimus also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, melatonin might increase levels of drugs metabolized by CYP3A4.
Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the gist for your situation. Your kidney cancer medicine, temsirolimus (Torisel), is partly cleared by a liver enzyme called CYP3A4. Some lab studies hint that certain batches of melatonin can slow that enzyme down. If that happened in real life, temsirolimus could build up a bit and you might feel its side effects more strongly, things like mouth sores, rash, nausea, fatigue, higher blood sugar, or a greater chance of infection.

I want to be honest about how soft this evidence is. This is theoretical, based only on test-tube research, and it's rated a minor interaction. It hasn't been shown to cause real problems in people. Still, because temsirolimus is a serious chemo drug, please run any melatonin use past your oncology team or pharmacist first.
