Here's the gist for vincristine. This is a chemotherapy drug, and one of its known side effects is that it can affect the nervous system. In some people that may include lowering the threshold for seizures. The concern on file is theoretical: melatonin has been linked in a few reports to more frequent seizures in certain people, especially children with neurological problems. Stacked together, the idea is that they might add up and slightly raise seizure risk.

I want to be honest, this is based on how these substances work, not strong proof, and it's rated as possible. Still, since you're on serious cancer treatment, please loop in your oncology team or pharmacist before adding melatonin so they can okay it for your situation.

Vincristine Sulfate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, melatonin might increase levels of drugs metabolized by CYP3A4.
Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the short version: vincristine is a powerful chemotherapy drug that your liver clears partly through an enzyme called CYP3A4. Some lab studies hint that certain batches of melatonin can slow that same enzyme down. If that happened in real life, vincristine could linger in your body a bit longer, which in theory might mean stronger effects or more side effects like numbness or tingling in the hands and feet, constipation, or muscle weakness.

I want to be honest about the strength of this, though. This is only seen in test-tube and animal research, not in actual patients, so it's very theoretical and rated as minor. Still, because vincristine is serious chemo, please don't add melatonin without first checking with your oncology team or pharmacist. They know your exact regimen and can give you the safest answer.
