Acetaminophen, Butalbital, Codeine Phosphate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group N,n-dmpea interacts with.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with this one. The codeine in your Phrenilin needs an enzyme called CYP2D6 to turn into morphine, which is the part that actually relieves pain. Codeine is a prodrug, meaning it has to be switched on by your body. N,N-DMPEA appears to block CYP2D6, at least in lab tests, so in theory it could leave more of your codeine stuck in its inactive form. That usually means less pain relief, not more, so your headache medicine might just work poorly.
I'll be straight with you, though: this is based only on test-tube data, so it's more caution than alarm. Since people respond very differently, it's worth a quick chat with your pharmacist before combining them.
