Acetaminophen, Codeine, Doxylamine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group N,n-dmpea interacts with.

In vitro research shows that N,N-DMPEA strongly inhibits cytochrome P450 2D6 (CYP2D6). Theoretically, concomitant use with drugs metabolized by CYP2D6 might increase the risk for adverse effects from these drugs.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the wrinkle with this combo: the codeine in Mersyndol is actually a prodrug. Your body has to use an enzyme called CYP2D6 to turn codeine into morphine, which is what actually relieves your pain. Lab studies suggest N,N-DMPEA can strongly block that same enzyme. So in theory, if this enzyme is slowed down, less of your codeine gets converted to morphine, and you may get weaker pain relief than expected.

I'll be honest though, this is based only on test-tube research, not real people, so we can't say how big a deal it is in practice. The acetaminophen and doxylamine parts aren't really affected here. Since N,N-DMPEA itself is poorly studied and a bit risky, it's worth running this past your pharmacist or doctor before mixing the two.
