Aluminum Hydroxide, Aspirin, Codeine Phosphate, Magnesium Hydroxide falls under CNS DEPRESSANTS — the group Pinellia Ternata interacts with.

Evidence from animal research shows that a Pinellia ternata preparation decreases activity and increases sleeping time. Theoretically, Pinellia ternata can potentiate the therapeutic effect of CNS depressants. Some of these medications include antihistamines, barbiturates, benzodiazepines, tricyclic antidepressants, and others.

Out of everything in this combination pill, the piece that matters here is the codeine. Codeine is an opioid that calms the brain and can make you drowsy. Pinellia ternata, based only on animal studies, seemed to lower activity and lengthen sleep, so the worry is that it could add to codeine's calming effect.

If both stack up, you might feel more sleepy, foggy, dizzy, or notice slower, shallower breathing, plus the usual codeine constipation. Honestly though, this is a theoretical concern from lab and animal work, not proven in people, so I wouldn't panic. The aspirin and the antacids (aluminum and magnesium) aren't part of this issue. Just check with your pharmacist or doctor before pairing them, especially if you already feel drowsy on this medicine.
