Alogliptin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rooibos interacts with.

Theoretically, taking rooibos with CYP3A4 substrates may increase the effects of CYP3A4 substrates.
In vitro research shows that the methanol extract of rooibos leaves and stems strongly inhibits CYP3A4 enzyme activity.

Here's the good news first: drinking rooibos tea alongside your alogliptin (Nesina) is very unlikely to cause a problem. The concern on file is purely theoretical. In a lab dish, a concentrated rooibos extract slowed down an enzyme called CYP3A4. If that happened in your body, drugs cleared by that enzyme could build up a little and have a stronger effect.

The catch is that alogliptin isn't heavily dependent on CYP3A4. It's mostly removed unchanged by your kidneys, so this pathway matters less for it than for many other drugs. A normal cup or two of tea shouldn't move your blood sugar around. If you're using concentrated rooibos capsules or extracts, just mention it to your pharmacist or doctor so they can keep an eye on things.

Alogliptin is not used for type 1 diabetes or diabetic ketoacidosis, as it would not be effective in those situations.
