Codeine Phosphate falls under CNS DEPRESSANTS — the group Tianeptine interacts with.

Tianeptine has sedative effects; high doses may have additive effects with other CNS depressants.
Higher doses of tianeptine have opioid like effects and may have additive effects with other sedative drugs. In one case report, a 42-year-old male presented with respiratory depression and aspiration pneumonia due to concomitant use of high-dose tianeptine and alprazolam.

Here's the concern with pairing these two: codeine is an opioid pain reliever that slows down your brain and breathing on its own. Tianeptine, especially in the high doses found in those over-the-counter 'gas station' products, also acts a lot like an opioid. Stack them together and the sedative effects can pile up. That means more drowsiness, foggy thinking, and in the worst case, dangerously slow or shallow breathing.

This isn't proven in big studies, it's based on how both drugs work, plus a case where someone combined high-dose tianeptine with a sedative and ended up with serious breathing trouble. Honestly, I'd steer clear of these tianeptine products altogether. Please talk with your doctor or pharmacist before mixing anything with your codeine.

Brand name: Codeine

Codeine Sulfate tablets are used for the management of mild to moderate pain in adults and adolescents when other treatments have not worked well enough or are not tolerated. It is an opioid medication, so it is only appropriate when non-opioid options are inadequate.

Because of its serious risks — including addiction, misuse, and overdose — codeine is reserved for patients who truly need an opioid. Prescribers are expected to use the lowest effective dose for the shortest time possible.
