Fentanyl citrate falls under CNS DEPRESSANTS — the group Tianeptine interacts with.

Tianeptine has sedative effects; high doses may have additive effects with other CNS depressants.
Higher doses of tianeptine have opioid like effects and may have additive effects with other sedative drugs. In one case report, a 42-year-old male presented with respiratory depression and aspiration pneumonia due to concomitant use of high-dose tianeptine and alprazolam.

Here's the big concern with this pairing: fentanyl is one of the strongest opioids we have, and it already slows your breathing and makes you very drowsy. Tianeptine, especially at the high doses found in those gas station products, acts like an opioid too. Stacking the two means their sedating effects can pile on top of each other.

In plain terms, you could end up with dangerously slow or shallow breathing, heavy sedation, confusion, or even loss of consciousness. There's a real case where someone combining high-dose tianeptine with another sedative ended up with respiratory depression and pneumonia. This warning is based on how these drugs work rather than big studies, but with fentanyl the stakes are simply too high. Please talk with your doctor or pharmacist before mixing anything with fentanyl.
