Tianeptine and Pentazocine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CNS DEPRESSANTS

Why this applies to Pentazocine

Pentazocine falls under CNS DEPRESSANTS — the group Tianeptine interacts with.

Tianeptine has sedative effects; high doses may have additive effects with other CNS depressants.
Higher doses of tianeptine have opioid like effects and may have additive effects with other sedative drugs. In one case report, a 42-year-old male presented with respiratory depression and aspiration pneumonia due to concomitant use of high-dose tianeptine and alprazolam.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the concern with pairing tianeptine and pentazocine (Talwin): both can slow down your central nervous system, and at higher doses tianeptine starts acting a lot like an opioid itself. Stack that on top of a true opioid pain reliever like pentazocine and the calming, sedating effects can pile up. That means more drowsiness, dizziness, confusion, and in a worst case, dangerously slow or shallow breathing.

I'll be honest with you, this is based mostly on how these drugs behave (pharmacology) rather than lots of human studies, but a real case report tied high-dose tianeptine plus a sedative to serious breathing problems. Tianeptine isn't FDA-approved here and the gas station versions are risky on their own. Please loop in your doctor or pharmacist before combining anything with your pentazocine.

From our pharmacist’s overview on the Pentazocine page

“Pentazocine (Talwin) is an opioid painkiller, so what matters most on this list is anything that deepens its sedation or slows your breathing, and here the risk is real enough to demand respect. Seventy-eight of the 167 supplement and herbal ingredients with an interaction on record are sedating in their own right, and six of the seven…”

About Tianeptine

What is Tianeptine?

  • Depression (prescription use in some countries)
  • Anxiety
  • Mood enhancement
  • Pain relief
  • Self-treatment of opioid withdrawal
  • Recreational euphoria

Tianeptine is a prescription antidepressant in some countries, but it is NOT approved by the FDA in the United States and is illegally sold in dietary supplements and 'gas station' products. At high doses it acts like an opioid and carries serious risks of addiction, overdose, and dangerous withdrawal. We strongly advise against using over-the-counter tianeptine products and urge anyone considering it to talk with a doctor or pharmacist first.
