CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Aspirin, Caffeine, Codeine Phosphate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Turmeric interacts with.
In vitro and animal research show that turmeric and its constituents curcumin and curcuminoids inhibit CYP3A4. Also, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking turmeric and cancer medications that are CYP3A4 substrates, including everolimus, ruxolitinib, ibrutinib, and palbociclib, and bortezomib. In another case report, a transplant patient presented with acute nephrotoxicity and elevated tacrolimus levels after consuming turmeric powder at a dose of 15 or more spoonfuls daily for ten days prior. It was thought that turmeric increased levels of tacrolimus due to CYP3A4 inhibition.
Conversely, other in vitro research suggests that turmeric induces CYP3A4 activity, leading to reduced levels of CYP3A4 substrates. An animal model suggests that induction of CYP3A4 occurs after daily curcumin use for 1 week. However, the induction of CYP3A4 by turmeric has not been reported in humans.

This combination product pairs aspirin and caffeine with codeine, and it's the codeine part that turmeric could touch. Codeine is a prodrug, meaning your body has to convert it into morphine to get pain relief. Some of that processing involves CYP3A4, the same enzyme turmeric may affect. The tricky thing is the lab data point in both directions: turmeric might slow this enzyme down or speed it up, so the effect on your pain relief isn't predictable.
Here's the honest part. This is based mostly on test-tube and animal studies, not solid human evidence. Sprinkling turmeric on your food almost certainly won't matter. Concentrated curcumin capsules are more of a question mark. If you take this medication regularly, just check with your pharmacist before adding a turmeric supplement.

ANTICOAGULANT/ANTIPLATELET DRUGS

Curcumin, a constituent of turmeric, has demonstrated antiplatelet effects in vitro. Furthermore, two case reports have found that taking turmeric along with warfarin or fluindione was associated with an increased international normalized ratio (INR). However, one clinical study in healthy volunteers shows that taking curcumin 500 mg daily for 3 weeks, alone or with aspirin 100 mg, does not increase antiplatelet effects or bleeding risk. It is possible that the dose of turmeric used in this study was too low to produce a notable effect.

The piece of this combo pill that matters here is the aspirin. Aspirin already thins the blood by keeping platelets from sticking together. Turmeric, especially in concentrated supplement form, may have a mild blood-thinning effect of its own. Stacked on top of aspirin, that could nudge your bleeding risk up a little, think easier bruising, nosebleeds, or bleeding that takes longer to stop.
Honestly, the evidence is mixed. One study in healthy people taking curcumin with low-dose aspirin found no real change, so this is a possible concern, not a sure thing. Sprinkling turmeric on your food is very unlikely to matter. If you're taking high-dose curcumin capsules, just run it by your pharmacist or doctor first, and don't change your medicine on your own.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

In vitro and animal research show that the turmeric constituent, curcumin, inhibits CYP1A2. However, other in vitro research suggests that curcumin does not significantly affect CYP1A2.

This combo product pairs aspirin, caffeine, and codeine. The piece that matters here is the caffeine, which your body clears using a liver enzyme called CYP1A2. In lab and animal studies, curcumin (the active part of turmeric) may slow that enzyme down a little. If it did that in people, caffeine could hang around longer, so you might feel a bit more jittery, have trouble sleeping, or notice a faster heartbeat.
Here's the honest part: the evidence is weak and even conflicting, and this is rated a minor interaction. Everyday turmeric in food is very unlikely to do anything. If you take concentrated turmeric or curcumin capsules and notice extra restlessness, just check in with your pharmacist or doctor before changing anything.
