Why this applies to Losartan Potassium

Losartan Potassium falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Turmeric interacts with.

Turmeric might increase or decrease levels of drugs metabolized by CYP3A4.
In vitro and animal research show that turmeric and its constituents curcumin and curcuminoids inhibit CYP3A4. Also, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking turmeric and cancer medications that are CYP3A4 substrates, including everolimus, ruxolitinib, ibrutinib, and palbociclib, and bortezomib. In another case report, a transplant patient presented with acute nephrotoxicity and elevated tacrolimus levels after consuming turmeric powder at a dose of 15 or more spoonfuls daily for ten days prior. It was thought that turmeric increased levels of tacrolimus due to CYP3A4 inhibition.
Conversely, other in vitro research suggests that turmeric induces CYP3A4 activity, leading to reduced levels of CYP3A4 substrates. An animal model suggests that induction of CYP3A4 occurs after daily curcumin use for 1 week. However, the induction of CYP3A4 by turmeric has not been reported in humans.

Here's something interesting about losartan: it's actually a prodrug. Your body uses a liver enzyme (CYP3A4, along with another called 2C9) to turn losartan into its more powerful active form, which does most of the blood-pressure-lowering work. Turmeric may nudge that same enzyme either direction in lab and animal studies.

If turmeric slows that enzyme, your body could make less of the active form, so losartan might not control your blood pressure as well. If it speeds the enzyme up, you could get a bit more effect, possibly feeling dizzy or lightheaded. Honestly, this is theoretical and based on weak evidence, and cooking with turmeric is very unlikely to matter. Concentrated curcumin capsules are a different story, so check with your pharmacist before starting them.

Here's the gist with turmeric and your losartan (Cozaar): the concern is that turmeric might gently boost losartan's blood-pressure-lowering effect. I want to be honest about where this comes from, though. It's based on a study in rats, not people, so it's really more of a theoretical heads-up than a proven problem.

If it did add up, the thing to watch for would be your pressure dropping a little lower than usual. That can show up as feeling dizzy, lightheaded, or unsteady, especially when you stand up quickly. For most folks, a sprinkle of turmeric in cooking won't budge anything. Concentrated curcumin capsules are the bigger question mark. If you're using those daily, mention it to your pharmacist or doctor so they can keep an eye on your numbers.

What Is Losartan Used For?

Brand name: Cozaar

Losartan tablets — available as branded Cozaar and several generic products — are used to treat high blood pressure (hypertension) in adults and children 6 years and older. Lowering blood pressure reduces the risk of strokes and heart attacks. Losartan tablets are also used to reduce the risk of stroke in people with high blood pressure who also have left ventricular hypertrophy (an enlarged left side of the heart). Note: evidence suggests this stroke-risk benefit may not apply to Black patients.

Losartan tablets are additionally approved to protect the kidneys in people with type 2 diabetes, high blood pressure, and signs of kidney damage (diabetic nephropathy). Arbli, the oral suspension form, shares these same approved uses.
