Vinpocetine and Glipizide, Metformin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Why this applies to Glipizide, Metformin

Glipizide, Metformin falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Vinpocetine interacts with.

Theoretically, vinpocetine might increase levels of drugs metabolized by CYP2C9.
In vitro research shows that vinpocetine weakly inhibits CYP2C9. However, this effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for this combo pill. The glipizide part is cleared by a liver enzyme called CYP2C9. In lab dish studies, vinpocetine seems to weakly slow that enzyme down. If that happened in your body, glipizide could hang around a bit longer and push your blood sugar lower than expected. The metformin side isn't really involved in this particular interaction.

The important thing: this is only test-tube data, and it has never actually been seen in real people. So I wouldn't lose sleep over it. Still, since glipizide can cause low blood sugar (shakiness, sweating, dizziness, confusion), it's smart to watch for those signs and check your readings if you add vinpocetine. Please run it by your pharmacist or doctor first.

From our pharmacist’s overview on the Glipizide, Metformin page

“Glipizide and metformin (Metaglip) already work hard to pull blood sugar down, which is exactly why this list of 307 ingredients deserves genuine vigilance rather than a shrug. Most of them, 239, can lower glucose on their own, and stacked on this pill the combined effect can tip into true hypoglycemia.”
