Diindolylmethane is generally well tolerated short-term, but long-term safety isn't well established. Common side effects include diarrhea, gas, headache, nausea, rash, and vomiting; a rare serious side effect is DRESS (drug rash with eosinophilia and systemic symptoms).

Fenugreek seed is generally well tolerated in food amounts, though supplement doses may cause abdominal pain, bloating, diarrhea, indigestion, gas, and nausea — and rare severe allergic reactions including swelling, wheezing, and shock have been reported. Indole-3-carbinol seems well tolerated but may cause diarrhea, imbalance, nausea, rash, tremor, and unsteadiness, especially at higher doses.

Tribulus is often well tolerated short-term in healthy adults, though rare cases of liver and kidney injury, seizures, and painful erection have been reported. In a randomized, double-blind, placebo-controlled study of 112 healthy volunteers, subjects taking the GNC vitamin and mineral blends for six weeks experienced significant improvements in serum levels of certain key nutrients compared to a placebo and a basic multivitamin formula based upon multivariate statistical analyses of a group of B vitamins (thiamin, niacin, riboflavin, pantothenic acid, biotin, folic acid, and vitamins B-6 and B-12) and key antioxidants and carotenoids (a group of beta-carotene, alpha-tocopherol, selenium, lutein and lycopene). Statistical improvements in SF-36 Vitality and Mental Health scores were also observed compared to placebo.

Theoretically, fenugreek might have additive effects when used with anticoagulant or antiplatelet drugs.
Some of the constituents in fenugreek have antiplatelet effects in animal and in vitro research. However, common fenugreek products might not contain sufficient concentrations of these constituents for clinical effects. A clinical study in patients with coronary artery disease or diabetes shows that taking fenugreek seed powder 2.5 grams twice daily for 3 months does not affect platelet aggregation, fibrinolytic activity, or fibrinogen levels .

Eurycoma longifolia can reduce the levels and clinical effects of propranolol.
A small clinical study in healthy persons shows that taking a single dose of a water-based Eurycoma longifolia extract 200 mg, in combination with a single dose of propranolol 80 mg, reduces the propranolol area under the curve (AUC) by 29%, reduces the peak concentration by 42%, and increases time to peak concentration by 86% when compared with control. Since the elimination half-life of propranolol did not change, it seems that Eurycoma longifolia alters the kinetics of propranolol by decreasing its absorption in the gut, and not by altering its metabolism. It is not known if separating administration will prevent this interaction.
