Abrocitinib falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafinil might increase the levels and clinical effects of CYP2C19 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to inhibit CYP2C19 activity in vitro.

Here's the gist for these two together. When your liver turns adrafinil into modafinil, that modafinil can act like a brake on a liver enzyme called CYP2C19. Your eczema medicine, abrocitinib (Cibinqo), leans on that same enzyme to be cleared from your body. So in theory, slowing that enzyme could let abrocitinib build up a bit higher than usual.

If that happened, you might notice more of its known effects, things like nausea, headaches, or a greater dip in your blood counts and a higher infection risk, since abrocitinib already quiets the immune system. I'll be honest though: this is a theoretical concern based on lab data, not real-world reports, so it may not amount to much. Still, please run it by your doctor or pharmacist before combining them.

Here's the gist for Cibinqo (abrocitinib): your body partly clears it through a liver enzyme called CYP2C9. Adrafinil turns into modafinil in your body, and lab studies hint that modafinil may slow that same enzyme down. If that happened, abrocitinib could clear a little slower and build up, which in theory could mean more of its effects and side effects, things like nausea, headache, acne flares, or a higher risk of infection.

I want to be honest that this is theoretical, based only on test-tube data, not real patient reports, so the actual risk is uncertain. Abrocitinib is already a closely monitored medication, so don't add adrafinil on your own. Check with your doctor or pharmacist first.

About Abrocitinib

What Is Abrocitinib Used For?

Brand name: Cibinqo

Cibinqo (abrocitinib) is used to treat moderate-to-severe atopic dermatitis (eczema) in adults and children 12 years of age and older. It is specifically for people whose eczema has not been adequately controlled with other systemic (whole-body) treatments, including biologics, or when those treatments are not a safe option.

Cibinqo is not recommended to be used at the same time as other JAK inhibitors, biologic immunomodulators, or other immunosuppressants (medicines that reduce immune system activity).
