Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP1A2 activity in vitro.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this combo. Adrafinil turns into modafinil in your body, and modafinil may gently speed up a liver enzyme called CYP1A2. If a medicine is cleared by that enzyme, revving it up could lower the drug's levels a bit. In your Tylenol Sinus Nighttime product, that mostly matters for the acetaminophen (Tylenol) piece, which CYP1A2 plays a small role in processing.
Honestly, this one is theoretical and only based on lab data, so I wouldn't lose sleep over it. The realistic concern isn't weaker pain relief, it's that acetaminophen's pathway can shift in ways that may stress the liver, and adrafinil is already hard on the liver. Since both can tax the liver, it's worth a quick chat with your pharmacist before pairing them, especially if you use them often.
