These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Adrafinil interacts with whole categories of medication. Acetaminophen, Orphenadrine falls into 2 of those categories, so it can interact with Adrafinil in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
1 of 2Why this applies to Acetaminophen, Orphenadrine
Acetaminophen, Orphenadrine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Adrafinil interacts with.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP1A2 activity in vitro.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your combination pill of acetaminophen and orphenadrine. Adrafinil is converted by your liver into modafinil, and modafinil can mildly speed up an enzyme called CYP1A2. Some of this medicine is handled by that same enzyme, so in theory adrafinil could nudge those levels a little lower. Practically, that might mean slightly less pain and muscle-relaxing relief than you'd expect.
I want to be honest: this is a theoretical concern based on how the drugs behave in the lab, not real-world reports of people having problems. The effect, if any, is likely small. Still, since adrafinil is unregulated and rough on the liver (and acetaminophen is too), please run this past your pharmacist or doctor before mixing them.

CYTOCHROME P450 2B6 (CYP2B6) SUBSTRATES
2 of 2Acetaminophen, Orphenadrine also falls under CYTOCHROME P450 2B6 (CYP2B6) SUBSTRATES.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP2B6 activity in vitro.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this combo. Adrafinil turns into modafinil in your body, and modafinil can mildly speed up a liver enzyme called CYP2B6. The orphenadrine part of your pill (a muscle relaxer that also eases pain) is handled in part by that enzyme. So in theory, adrafinil could nudge your body to clear orphenadrine a little faster, which might make it work less well. You may notice your muscle aches or stiffness aren't as well controlled.
I want to be straight with you, though: this is based on lab and pharmacology reasoning, not real-world cases, so the effect (if any) is likely small. The acetaminophen side isn't really the concern here. Before mixing these, please check with your pharmacist or doctor.

“Orfenagesic combines acetaminophen with orphenadrine, a muscle relaxant, and the acetaminophen half is what makes its interaction list worth real vigilance. Six of the 160 supplement ingredients on file carry a major rating, and the largest risky cluster, 38 ingredients including beer and wine, adds to the strain acetaminophen already…”
