Adrafinil and Acetaminophen, Phenylephrine, Chlorpheniramine

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Adrafinil interacts with whole categories of medication. Acetaminophen, Phenylephrine, Chlorpheniramine falls into 3 of those categories, so it can interact with Adrafinil in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 3

Why this applies to Acetaminophen, Phenylephrine, Chlorpheniramine

Acetaminophen, Phenylephrine, Chlorpheniramine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafinil might reduce the levels and clinical effects of CYP1A2 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP1A2 activity in vitro.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the honest picture for this combo. Adrafinil turns into modafinil in your body, and modafinil may slightly speed up a liver enzyme called CYP1A2. In theory, that faster clearance could lower the levels of medicines this enzyme handles. With your Super Cold Tabs, though, the three ingredients (acetaminophen for pain and fever, phenylephrine as a decongestant, and chlorpheniramine as an antihistamine) aren't mainly cleared by CYP1A2, so any real-world effect on how well they work is likely small.

This warning is theoretical, based on lab data, not patient reports, so I wouldn't lose sleep over it. The bigger concern with adrafinil itself is liver strain, and acetaminophen is also processed by the liver. To be safe, please run this past your pharmacist or doctor before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 3

Acetaminophen, Phenylephrine, Chlorpheniramine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.

Here's the gist with this combo cold tablet. Adrafinil turns into modafinil in your body, and modafinil can mildly speed up a liver enzyme called CYP3A4. Some of the ingredients in Super Cold Tabs are handled by that same enzyme, so in theory your body might clear them a touch faster. That could mean slightly weaker or shorter relief, for example the chlorpheniramine (the antihistamine for sneezing and runny nose) or the phenylephrine decongestant not working quite as well.

Honestly, this is a theoretical concern based on how the drugs behave, not something proven in real patients, and modafinil's effect on this enzyme is weak. I wouldn't lose sleep over it. Still, since adrafinil is unregulated and tough on the liver, run it by your pharmacist or doctor first.

STIMULANT DRUGS

3 of 3

Acetaminophen, Phenylephrine, Chlorpheniramine also falls under STIMULANT DRUGS.

Theoretically, taking adrafinil with stimulant drugs might have additive stimulant effects.
Adrafinil is a precursor to the prescription drug modafinil, which has stimulant effects.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's what to keep in mind with this combo. Super Cold Tabs contains phenylephrine, a decongestant that works like a mild stimulant on your body. Adrafinil turns into modafinil in your liver, and modafinil also has stimulant-like, wakefulness-boosting effects. Stack the two together and those effects can add up. This isn't a deadly chemical reaction, it's more of an overlap, but it could leave you feeling jittery, restless, or with a faster heartbeat, higher blood pressure, or trouble sleeping.

To be honest, this concern is theoretical, based on how these compounds work rather than solid studies. Still, if you have heart issues or high blood pressure, it's worth a quick chat with your pharmacist before pairing them.
