Berotralstat Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Adrafinil interacts with.

Theoretically, drugs that inhibit CYP3A4 might increase the levels and clinical effects of adrafinil.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil is metabolized by CYP3A4.

Here's the thing to keep in mind if you take Orladeyo (berotralstat), which is used to prevent attacks of hereditary angioedema. Berotralstat is known to block an enzyme called CYP3A4. Adrafinil turns into modafinil in your body, and modafinil is cleared by that same CYP3A4 enzyme. So in theory, slowing that enzyme could let modafinil build up a bit, meaning stronger or longer-lasting stimulant effects.

In real life that might look like more trouble sleeping, jitteriness, a faster heartbeat, headache, or anxiety. I'll be honest, this is a theoretical concern based on how the drugs work, not something proven in studies, so I don't want to alarm you. Still, since adrafinil is unregulated and hard on the liver, please run this combination by your doctor or pharmacist first.
