Dolutegravir, Rilpivirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Here's the concern with pairing Adrafinil and Juluca (dolutegravir plus rilpivirine). Once your liver turns adrafinil into modafinil, modafinil can mildly speed up an enzyme called CYP3A4. Rilpivirine, one of the two HIV medicines in Juluca, is cleared by that same enzyme. So in theory, revving up CYP3A4 could lower rilpivirine levels in your blood and make it work less well.

That matters more than usual here, because with HIV therapy you want steady, reliable drug levels. If levels dip too low, the virus has more room to rebound or develop resistance. To be fair, this is a theoretical concern based on pharmacology, not proven cases, and the effect is considered weak. Still, please check with your doctor or pharmacist before combining them.
