Adrafinil + drospirenone, ethinyl estradiol, levomefolate
drospirenone, ethinyl estradiol, levomefolate brand name: Beyaz.
drospirenone, ethinyl estradiol, levomefolate falls into 3 drug categories that Adrafinil can interact with; the most serious is rated moderate. Theoretically, adrafinil might reduce the levels and clinical effects of CYP1A2 substrates.

These two interact in 3 distinct ways. Each one is explained separately below.
Why Are There 3 Interactions?
Adrafinil interacts with whole categories of medication. drospirenone, ethinyl estradiol, levomefolate falls into 3 of those categories, so it can interact with Adrafinil in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
1 of 3Why this applies to drospirenone, ethinyl estradiol, levomefolate
drospirenone, ethinyl estradiol, levomefolate falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Adrafinil interacts with.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP1A2 activity in vitro.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version about pairing adrafinil with Beyaz (a birth control pill that contains drospirenone, ethinyl estradiol, and a folate). Once your liver turns adrafinil into modafinil, that modafinil may mildly speed up an enzyme called CYP1A2, which is one of the pathways that helps process the estrogen part of your pill. In theory, that could lower the hormone levels a little.
I want to be honest: this is a theoretical concern based on lab data, not real-world failures, and Beyaz mostly relies on other enzymes (CYP3A4). Still, with birth control, even a small dip raises the worry of breakthrough bleeding or reduced protection. Please talk with your pharmacist or doctor before combining them, and ask about a backup method just in case.

CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS
2 of 3drospirenone, ethinyl estradiol, levomefolate also falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil is metabolized by CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Beyaz (drospirenone, ethinyl estradiol, plus the folate). The estrogen part of your pill mildly blocks a liver enzyme called CYP3A4, which is the same enzyme that turns adrafinil into its active form, modafinil. On paper, that could let a little more modafinil build up, so you might feel jittery effects more strongly: trouble sleeping, a racing heart, headache, or feeling wired.
This is a theoretical concern based on how the drugs are processed, not real-world reports, so I don't want you to panic. Honestly, the bigger conversation is that adrafinil is unregulated and can stress your liver. Please loop in your doctor or pharmacist before pairing these two.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
3 of 3drospirenone, ethinyl estradiol, levomefolate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with Beyaz (a birth control pill that combines drospirenone, ethinyl estradiol, and a folate). Once your liver turns adrafinil into modafinil, that modafinil can gently speed up an enzyme called CYP3A4, which is one of the main ways your body breaks down the hormones in this pill. If those hormones clear out a little faster, the pill could become slightly less reliable at preventing pregnancy, and you might notice some breakthrough bleeding or spotting.
I want to be honest that this is a theoretical concern based on how these drugs work, and modafinil is only a weak enzyme booster. Still, since the stakes (an unplanned pregnancy) are high, please talk with your doctor or pharmacist before combining them and ask if a backup method makes sense.
