Esterified Estrogens, Methyltestosterone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Here's the gist with Estratest, which combines esterified estrogens and methyltestosterone. Once adrafinil turns into modafinil in your liver, that modafinil can mildly speed up a liver enzyme called CYP3A4. Since your estrogen hormones are cleared by this same enzyme, revving it up could lower estrogen levels a bit, possibly making the medication work less well. You might notice hot flashes, night sweats, or other menopausal symptoms creeping back.

I want to be honest though: this is a theoretical concern based on how the drugs behave, not strong patient studies, and modafinil's effect on this enzyme is weak. Still, since adrafinil is unregulated and hard on the liver, please run it by your doctor or pharmacist before combining the two.
