Lacosamide falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafinil might increase the levels and clinical effects of CYP2C9 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to reduce CYP2C9 expression in vitro.

Here's the gist for Vimpat (lacosamide). Your liver uses an enzyme called CYP2C9 as one of the ways it breaks down lacosamide. Once your body turns adrafinil into modafinil, that modafinil may slow this same enzyme down. In theory, that could let lacosamide build up a bit higher than usual.

If levels climbed, you might notice more of lacosamide's typical effects, things like dizziness, drowsiness, blurry or double vision, headache, or feeling unsteady. I want to be honest with you though: this interaction is only theoretical, based on lab data, not real patient cases. Still, since adrafinil is unregulated and rough on the liver, please run it by your doctor or pharmacist before mixing the two.

Here's the gist for Vimpat (lacosamide). Adrafinil turns into modafinil in your liver, and modafinil can gently speed up an enzyme called CYP3A4. Since CYP3A4 is one of the pathways that helps clear lacosamide, the concern is that pairing the two might nudge your lacosamide levels down a little, which could make it slightly less effective at controlling your seizures.

I want to be honest about how soft this concern is. It's theoretical, based on how these drugs behave on paper, not from real patient reports. Lacosamide also relies on more than one enzyme, so any effect is likely small. Still, with a seizure medicine you don't want surprises. Please loop in your neurologist or pharmacist before adding adrafinil, and watch for any breakthrough seizures.

About Lacosamide

What Is Lacosamide Used For?

Brand names: Vimpat, Motpoly XR

Lacosamide is used to treat partial-onset seizures (seizures that start in one part of the brain) in adults and children. The age at which it is approved depends on the specific product: Vimpat tablets, oral solution, and injection are approved for partial-onset seizures in patients as young as 1 month old; most other formulations are approved from age 4 and up; and Motpoly XR is approved for patients weighing at least 50 kg.

Lacosamide is also used as adjunctive (add-on) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain) in adults and children 4 years of age and older, including Motpoly XR for patients weighing at least 50 kg.
