Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Here's the gist for combining adrafinil with Osphena (ospemifene). Your liver uses an enzyme called CYP3A4 to handle ospemifene. Adrafinil turns into modafinil in the body, and modafinil can mildly speed up that same enzyme. If the enzyme runs faster, your body may clear ospemifene a bit quicker, which could make it work less well for vaginal dryness and painful intercourse.

I want to be honest about how soft this is: it's a theoretical concern based on how these drugs behave, not on real-world reports, and modafinil is only a weak booster of this enzyme. Still, since adrafinil is unregulated and hard on the liver, please check with your pharmacist or doctor before pairing the two.

What Is Ospemifene Used For?

Brand name: Osphena

Osphena (ospemifene) is used to treat two specific symptoms of vulvar and vaginal atrophy — the thinning and drying of vaginal tissues that can happen after menopause. The first is moderate to severe dyspareunia (painful intercourse), and the second is moderate to severe vaginal dryness.

Both of these uses are for postmenopausal women only. Osphena is not used in children or in women who are pregnant or may become pregnant.
