Sorafenib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Here's the basic idea with Sorafenib (Nexavar) and adrafinil. Once your liver turns adrafinil into modafinil, that modafinil can gently speed up a liver enzyme called CYP3A4. Sorafenib is partly handled by that same enzyme, so in theory a faster enzyme could clear sorafenib a little quicker and leave somewhat lower levels in your body. With a cancer medicine, the worry isn't more side effects, it's that the drug might not work as well as it should.

I want to be honest about the strength of this: it's theoretical, based on how the drugs behave on paper, and modafinil is only a weak enzyme booster. Still, because sorafenib is treating something serious, please don't add adrafinil without first checking with your oncologist or pharmacist.

About Sorafenib

What Is Sorafenib Used For?

Brand name: Nexavar

Sorafenib tablets are approved to treat three types of cancer. The first is unresectable hepatocellular carcinoma (HCC) — liver cancer that cannot be removed with surgery. The second is advanced renal cell carcinoma (RCC) — kidney cancer that has spread or progressed. Both the brand Nexavar and generic sorafenib tablets are approved for these two uses.

The third use is for locally recurrent or metastatic, progressive differentiated thyroid carcinoma (DTC) — thyroid cancer that has spread or worsened and no longer responds to radioactive iodine treatment. Nexavar and sorafenib tablets are both approved for this use as well.
