Trimethaphan falls under ANTIHYPERTENSIVE DRUGS — the group Adrafinil interacts with.

Theoretically, adrafinil might reduce the effects of antihypertensive drugs.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to increase diastolic and systolic blood pressure. Retrospective analyses show that patients taking modafinil had a small but increased rate of requiring new antihypertensive drugs or increased use of antihypertensive drugs when compared with placebo.

Here's the gist: once your liver turns adrafinil into modafinil, that active form tends to nudge your blood pressure up a bit. Trimethaphan (Arfonad) is a strong IV blood pressure lowering medicine, used in a hospital to bring high pressure down quickly. So in theory, adrafinil could push against what trimethaphan is trying to do, making it harder to get your blood pressure to the target.

I'll be honest, this concern is theoretical, based on how these drugs work rather than real cases together, and trimethaphan is given in carefully monitored settings. Still, please don't combine them without telling your care team. Talk with your doctor or pharmacist first.
