Lepirudin is a powerful injectable blood thinner, usually given in the hospital to people who can't take heparin. It works directly to stop clots from forming. Here's the concern: lab and animal studies hint that andrographis may keep your platelets (the cells that help blood clot) from sticking together. Stack that mild blood-thinning effect on top of an already strong anticoagulant, and you could tip toward too much bleeding, like easy bruising, nosebleeds, bleeding gums, or in serious cases internal bleeding.

I want to be straight with you, though: this interaction is theoretical and only comes from test-tube and animal data, not human studies. Since lepirudin is closely monitored in a hospital setting, mention any supplements to your care team and let them make the call.
