This combination pill packs two ingredients that already make you sleepy: butalbital (a barbiturate) and codeine (an opioid). Ashwagandha is thought to have its own calming, sedative-like effect. Put them together and the worry is simply additive drowsiness, not a chemical change in how the drug is broken down.

What that could feel like is more grogginess, dizziness, slowed thinking, or feeling unusually sleepy, and in larger amounts, slowed breathing from the codeine and butalbital side. Keep in mind this concern is theoretical, based on how ashwagandha seems to act rather than firm studies. Still, since these effects can stack, please check with your pharmacist or doctor before pairing them, and avoid driving until you know how you feel.

Here's the concern with this combo: one of the ingredients in this medicine, acetaminophen (Tylenol), is hard on the liver if you take too much or take it for a long time. Ashwagandha has, in rare cases, been tied to liver problems of its own, from mild inflammation to more serious injury. So the worry is that the two together could add up and stress your liver more than either one alone.

To be honest, this is a theoretical caution based on how each one behaves, not on strong studies of the two taken together, so don't panic. Still, watch for signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness. Before starting ashwagandha alongside this medication, it's worth a quick chat with your pharmacist or doctor.

This combination pill has a lot going on, but the part that touches ashwagandha is the caffeine. Caffeine is cleared by a liver enzyme called CYP1A2, and lab studies (not human studies) hint that ashwagandha may speed that enzyme up a bit. In theory, that could clear caffeine slightly faster, so you might notice a little less of caffeine's usual lift or feel it wear off sooner.

Honestly, this is a very minor, theoretical concern based only on test-tube research, so I wouldn't lose sleep over it. The codeine, butalbital, and acetaminophen pieces aren't really affected by this particular interaction. If you take this medicine regularly, just run the ashwagandha by your pharmacist or doctor first.

This combination pill is a mix, and the part to think about here is the butalbital, which your liver clears partly through an enzyme called CYP3A4. Some early lab research hints that ashwagandha may speed that enzyme up a bit. In theory, that could clear butalbital a little faster and make the headache or tension relief feel slightly weaker. The codeine piece works differently (it relies on a separate enzyme to turn into its active form), so this particular interaction doesn't really apply to it.

I want to be honest: this is only based on test-tube and animal data, the rating is minor, and many people would notice nothing at all. Still, if you take this regularly, mention the ashwagandha to your pharmacist or doctor so they can keep an eye on how well your medicine is working.
