Ashwagandha and Alogliptin, Pioglitazone

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Ashwagandha interacts with whole categories of medication. Alogliptin, Pioglitazone falls into 3 of those categories, so it can interact with Ashwagandha in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ANTIDIABETES DRUGS

1 of 3

Why this applies to Alogliptin, Pioglitazone

Alogliptin, Pioglitazone falls under ANTIDIABETES DRUGS — the group Ashwagandha interacts with.

Theoretically, taking ashwagandha with antidiabetes drugs might increase the risk of hypoglycemia.
There is preliminary clinical evidence suggesting that ashwagandha might lower blood glucose levels. Theoretically, ashwagandha might have additive effects when used with antidiabetes drugs and increase the risk of hypoglycemia.
Severity Moderate Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Oseni combines two diabetes medicines: alogliptin, which boosts your body's own insulin signals, and pioglitazone, which helps your cells respond better to insulin. Both work to bring your blood sugar down. Ashwagandha may also nudge blood sugar a little lower on its own. Put them together and the effects could add up, so your readings might dip lower than you expect.

This is mostly a theoretical concern backed by early studies, so don't panic. But it's worth knowing the signs of low blood sugar: shakiness, sweating, dizziness, a racing heart, or sudden hunger. If you'd like to try ashwagandha, check with your pharmacist or doctor first, and keep an eye on your home glucose numbers when you start.

HEPATOTOXIC DRUGS

2 of 3

Alogliptin, Pioglitazone also falls under HEPATOTOXIC DRUGS.

Theoretically, taking ashwagandha with hepatotoxic drugs might increase the risk of liver damage.
Ashwagandha has been linked to cases of acute hepatitis, liver failure, hepatic encephalopathy, autoimmune hepatitis, the need for liver transplantation, and death due to liver failure.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the concern with combining ashwagandha and Oseni (alogliptin plus pioglitazone). This isn't about ashwagandha changing how your diabetes medicine works or affecting your blood sugar directly. Instead, it's about your liver. Pioglitazone is processed by the liver and, on rare occasions, has been linked to liver enzyme changes. Ashwagandha, in some reported cases, has also been tied to liver injury. Stacking two things that can stress the liver may, in theory, add up.

To be fair, this is a theoretical caution based on how these work, not proven cases of the two together. Still, watch for warning signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness. Please run this by your pharmacist or doctor before starting ashwagandha.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Alogliptin, Pioglitazone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Oseni combines two diabetes medicines, and the pioglitazone part is partly cleared by a liver enzyme called CYP3A4. Lab studies hint that ashwagandha may speed that enzyme up. In theory, that could clear pioglitazone a little faster and make it slightly less effective, which might show up as your blood sugar creeping higher than usual.

Here's the honest part: this is based only on test-tube research, not on real people, and it's rated a minor, possible interaction. For most folks it likely won't cause a noticeable change. Still, if you take Oseni and want to try ashwagandha, it's worth a quick chat with your pharmacist or doctor, and keep an eye on your blood sugar readings so nothing slips by.

From our pharmacist’s overview on the Alogliptin, Pioglitazone page

“Someone taking Oseni (alogliptin and pioglitazone) for type 2 diabetes often shops the very shelf this list comes from, and the pairing deserves genuine caution. Of the 356 ingredients on file, 239 also lower blood sugar, so stacking them on top of the pill can drop glucose too far.”
