Atazanavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ashwagandha interacts with.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.

Here's the gist with atazanavir (Reyataz). Your body relies heavily on an enzyme called CYP3A4 to handle this medicine. Lab studies hint that ashwagandha may speed up that enzyme, which in theory could clear atazanavir out of your system a little faster and lower its levels in your blood.

Why that matters: with HIV medicines, you want steady, reliable drug levels. If levels dip too low, the virus has more room to multiply, and that can risk treatment failure or resistance over time. To be clear, this concern comes only from test-tube data, not from people, so it's theoretical and rated minor. Still, because keeping HIV controlled is so important, I'd run ashwagandha by your pharmacist or HIV provider before starting it.

Ashwagandha is an Ayurvedic herb most often taken to help with stress, anxiety, and sleep, and some small studies suggest it may help, though the evidence is still limited. About Atazanavir

What Is Atazanavir Used For?

Brand name: Reyataz

Atazanavir capsules are used together with other antiretroviral medicines to treat HIV-1 infection in adults and in children 6 years and older who weigh at least 15 kg (about 33 lbs).

Reyataz — available as capsules and as an oral powder — is also approved for HIV-1 treatment in adults and in children as young as 3 months who weigh at least 5 kg (about 11 lbs). Reyataz oral powder must always be taken with ritonavir and food, and is not for use in children weighing less than 5 kg.
