Theoretically, taking ashwagandha might decrease the effects of immunosuppressants.
Ashwagandha has demonstrated immunostimulant effects in humans. Animal research has shown that ashwagandha can attenuate the immunosuppression caused by cyclophosphamide.

Here's the concern in plain terms: azathioprine works by quieting your immune system, which is exactly what you want when you're protecting a transplanted kidney or calming the joint attacks of rheumatoid arthritis. Ashwagandha, on the other hand, has been shown to stimulate the immune system. So in theory, taking the two together could push against each other, weakening how well azathioprine does its job. That might mean a higher risk of transplant rejection or a flare of your arthritis symptoms.

I'll be straight with you, though: this is a theoretical worry based mostly on lab and animal studies, not solid human evidence with these two together. Still, because azathioprine is such an important medication, I'd skip the ashwagandha unless your transplant team or doctor signs off first.

Here's the concern with pairing these two. Azathioprine is known to be hard on the liver, and it's one of the reasons your doctor checks your liver enzymes with regular blood work while you're on it. Ashwagandha, though uncommon, has been tied to liver problems in a handful of reported cases, ranging from mild liver inflammation to more serious damage. Stacking two things that can stress the liver may add up.

I want to be honest that this is a theoretical concern based on how each one behaves, not a proven, common reaction. Still, watch for warning signs like yellowing skin or eyes, dark urine, belly pain, unusual tiredness, or nausea. Please loop in your pharmacist or transplant or rheumatology team before adding ashwagandha.

Ashwagandha is an Ayurvedic herb most often taken to help with stress, anxiety, and sleep, and some small studies suggest it may help, though the evidence is still limited. It is generally well tolerated for short-term use in healthy adults, but it should be avoided in pregnancy and used cautiously by people with thyroid or liver problems. Talk with your pharmacist or doctor before starting it.

About Azathioprine

What Is Azathioprine Used For?

Brand names: Imuran, Azamune

Azathioprine is used alongside other medications to help prevent the body from rejecting a kidney transplant (renal homotransplantation). It is also used to treat active rheumatoid arthritis — reducing signs and symptoms in adults who have not responded well enough to other treatments.

Other pain relievers such as aspirin, NSAIDs, or low-dose steroids may be continued alongside azathioprine for rheumatoid arthritis. Combining azathioprine with disease-modifying antirheumatic drugs (DMARDs) has not been well studied and is not recommended.
