Theoretically, taking ashwagandha with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that ashwagandha might lower systolic and diastolic blood pressure. Theoretically, ashwagandha might have additive effects when used with antihypertensive drugs and increase the risk of hypotension.

Here's the thing to keep in mind with this pairing. Bosentan works in your lungs to relax those narrowed blood vessels, but it can also gently lower your overall blood pressure as a side effect. Ashwagandha, based mostly on animal studies, may also nudge blood pressure down a bit. Put the two together and the worry is that they could stack up and drop your pressure a little too far.

If that happened, you might feel dizzy, lightheaded, or wobbly when standing up. I want to be honest though, this is a theoretical concern from lab and animal data, not strong human evidence. Still, since you're managing PAH, it's worth a quick chat with your pharmacist or doctor before adding ashwagandha.

Here's the thing about Bosentan (Tracleer): it already carries a well-known risk of stressing your liver. That's why your doctor checks your liver enzymes with blood tests before and during treatment. Adding ashwagandha into the mix is a concern because this herb has been tied to rare but real cases of liver injury, from hepatitis all the way up to liver failure.

The worry here is theoretical, based mostly on how each can affect the liver, not on studies of the two together. Still, stacking two liver stressors isn't a gamble I'd take lightly. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and please check with your pharmacist or doctor before starting ashwagandha.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.

Here's the gist with bosentan (Tracleer) and ashwagandha. Your liver uses an enzyme called CYP3A4 to clear bosentan from your body. Some early lab research hints that ashwagandha might speed up that enzyme, which in theory could lower bosentan levels and make it work a little less well for your pulmonary hypertension. Bosentan isn't a prodrug, so faster clearance means less effect, not more.

I want to be honest though: this is theoretical, based only on test-tube studies, and it's rated minor. We don't have real-world cases showing harm. Still, because keeping your lung pressures controlled matters, please run it by your pharmacist or doctor before adding ashwagandha.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
