These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Ashwagandha interacts with whole categories of medication. Ethinyl Estradiol, Norgestimate falls into 3 of those categories, so it can interact with Ashwagandha in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

HEPATOTOXIC DRUGS

1 of 3

Why this applies to Ethinyl Estradiol, Norgestimate

Ethinyl Estradiol, Norgestimate falls under HEPATOTOXIC DRUGS — the group Ashwagandha interacts with.

Theoretically, taking ashwagandha with hepatotoxic drugs might increase the risk of liver damage.
Ashwagandha has been linked to cases of acute hepatitis, liver failure, hepatic encephalopathy, autoimmune hepatitis, the need for liver transplantation, and death due to liver failure.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: your birth control pill (Ortho Tri-Cyclen and similar) contains estrogen and a progestin, and your liver does the work of processing these hormones. Ashwagandha, on rare occasions, has been tied to liver problems of its own. The worry here isn't that one changes how the other works, it's that both lean on your liver, so combining them could in theory add stress to that organ.

To be honest, this is a theoretical concern based on how things work, not from studies showing it actually happens with this pill. Most people tolerate both fine. Still, watch for warning signs like yellowing skin or eyes, dark urine, belly pain on your right side, or unusual tiredness. Before starting ashwagandha, run it by your pharmacist or doctor, especially if you've had any liver issues.

Ethinyl Estradiol, Norgestimate also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP1A2 substrates.

Here's the gist for your birth control pill (Ortho Tri-Cyclen and its generics). Ashwagandha may slightly speed up a liver enzyme called CYP1A2, which is one of the pathways that helps break down medications. If that happens, your body could clear the estrogen and progestin in your pill a bit faster, which in theory could weaken how well it works.

I want to be honest about the evidence though: this is only seen in lab and test-tube studies, not in real people, and it's rated as a minor, possible concern. So please don't panic or stop anything. Still, because reliable pregnancy prevention matters, it's worth a quick chat with your pharmacist or doctor before adding ashwagandha, and watch for any unexpected breakthrough bleeding.

Here's the short version on combining ashwagandha with your birth control pill (the ethinyl estradiol and norgestimate combo). Both of these hormones are processed by a liver enzyme called CYP3A4. Some early lab research hints that ashwagandha might speed that enzyme up, which could in theory clear the hormones out a little faster and slightly lower their levels. With a birth control pill, the worry there would be reduced protection or some breakthrough bleeding or spotting.

That said, this is very weak evidence, only seen in test-tube studies, not in real people, and it's rated as a minor interaction. I wouldn't lose sleep over it, but since pregnancy prevention matters, it's worth a quick chat with your pharmacist or doctor before starting ashwagandha.
