Theoretically, taking ashwagandha with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that ashwagandha might lower systolic and diastolic blood pressure. Theoretically, ashwagandha might have additive effects when used with antihypertensive drugs and increase the risk of hypotension.

Here's the gist: felodipine already lowers your blood pressure by relaxing your blood vessels. Some animal studies hint that ashwagandha may nudge blood pressure down too. Put them together and the worry is simply that the effects could stack, leaving your pressure a little lower than intended.

If that happened, you might feel dizzy, lightheaded, or unusually tired, especially when standing up quickly. I'll be honest though, this is mostly theoretical and comes from lab and animal work, not strong human studies, so for most people it's a minor concern. Still, if you're already running on the low side or you add ashwagandha, it's worth checking your blood pressure at home and giving your pharmacist or doctor a quick call before starting.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.

Here's the gist for these two together. Your body relies on a liver enzyme called CYP3A4 to clear felodipine out of your system. Some early lab research hints that ashwagandha may speed that enzyme up. If that happened in real life, felodipine could be broken down a little faster, meaning slightly less of it sticks around to relax your blood vessels. The practical worry would be a touch less blood pressure control.

I want to be honest about how thin this is. This is only a theoretical concern from test-tube studies, not from people, and it's rated minor. Felodipine is not a prodrug, so it doesn't flip the other way. If you decide to try ashwagandha, just keep an eye on your home blood pressure readings and check in with your pharmacist or doctor first.
