Ashwagandha and hydrocortisone, pyrilamine, chlorpheniramine

Here is the interaction we found between these two, with the evidence behind it.

Minor

Serotonergic Drugs

Why this applies to hydrocortisone, pyrilamine, chlorpheniramine

hydrocortisone, pyrilamine, chlorpheniramine falls under Serotonergic Drugs — the group Ashwagandha interacts with.

Some animal studies have reported that ashwagandha can enhance serotonergic transmission by altering certain serotonin (5-HT) receptors. However, there is no evidence to suggest that ashwagandha increases the risk of serotonin-related effects, and there have been no published case reports of serotonin syndrome when combined with other serotonergic drugs. Nevertheless, due to the lack of extensive studies on the matter and the fact that ashwagandha appears to affect serotonergic pathways, it would be prudent to exercise caution when combining it with drugs that affect serotonin. This particular product (HC Derma-Pax) combines hydrocortisone, pyrilamine, and chlorpheniramine, and it's usually applied to the skin for itching and irritation. The interaction on file is really about ashwagandha's possible gentle nudge on serotonin pathways, which lines up most with the antihistamine pieces here (chlorpheniramine and pyrilamine). The hydrocortisone part is a steroid and isn't really involved in this concern.

The honest bottom line: this is a minor, mostly theoretical interaction based on animal data. There are no real case reports of problems when ashwagandha is mixed with serotonin-affecting drugs. With a skin product, almost none of these ingredients reach your bloodstream, so the risk is very low. If you ever take an oral antihistamine alongside ashwagandha, just watch for extra drowsiness and check in with your pharmacist.
