Here is the interaction we found between these two, with the evidence behind it.
HEPATOTOXIC DRUGS
Why this applies to Methenamine Mandelate
Methenamine Mandelate falls under HEPATOTOXIC DRUGS — the group Ashwagandha interacts with.
Ashwagandha has been linked to cases of acute hepatitis, liver failure, hepatic encephalopathy, autoimmune hepatitis, the need for liver transplantation, and death due to liver failure.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with pairing ashwagandha and this medication: both can be tough on the liver. Ashwagandha has been linked in rare reports to liver problems, including hepatitis and, very uncommonly, serious liver injury. When you stack it with a drug that also carries a liver-strain risk, the worry is that the two together could add up and stress your liver more than either one alone.
I want to be honest, though: this is a theoretical interaction based on how these things behave, not on solid studies showing it happens. Still, it's worth watching for warning signs like yellowing of the skin or eyes, dark urine, belly pain, nausea, or unusual tiredness. Please check with your pharmacist or doctor before combining them, and don't stop anything on your own.

About Ashwagandha
What is Ashwagandha?
- Stress and anxiety
- Sleep problems
- Fatigue and low energy
- Athletic performance
- General well-being (adaptogen)
Ashwagandha is an Ayurvedic herb most often taken to help with stress, anxiety, and sleep, and some small studies suggest it may help, though the evidence is still limited. It is generally well tolerated for short-term use in healthy adults, but it should be avoided in pregnancy and used cautiously by people with thyroid or liver problems. Talk with your pharmacist or doctor before starting it.
About Methimazole
What Is Methimazole Used For?
Brand names: Mandelamine, Methimazole
Methimazole tablets are used in adults and children with hyperthyroidism (an overactive thyroid) due to Graves' disease or toxic multinodular goiter when surgery or radioactive iodine is not the right option. They work to lower thyroid hormone levels and ease symptoms.
Methimazole tablets are also used to bring thyroid hormone levels down before a patient undergoes thyroidectomy (thyroid surgery) or radioactive iodine treatment, making those procedures safer.
