Mometasone, formoterol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ashwagandha interacts with.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.

Zenhale is an inhaler that pairs mometasone (a steroid that calms inflammation in your airways) with formoterol (a medicine that relaxes the airways so you breathe easier). The body uses an enzyme called CYP3A4 to clear these medicines, especially the steroid portion. Lab studies suggest ashwagandha might speed that enzyme up a little, which could, in theory, lower how much of the inhaled steroid sticks around.

Here is the reassuring part: this idea comes only from test-tube research, not from people, and the concern is rated minor. With an inhaler, very little medicine reaches the rest of your body to begin with, so a real-world effect on your asthma control is unlikely. Still, if you notice more wheezing or shortness of breath after starting ashwagandha, mention it to your pharmacist or doctor before changing anything.
