Norelgestromin, Ethinyl Estradiol falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ashwagandha interacts with.
In vitro research shows that ashwagandha extract induces CYP1A2 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version on pairing ashwagandha with your Ortho Evra patch (norelgestromin plus ethinyl estradiol). In lab dishes, ashwagandha appears to speed up an enzyme called CYP1A2, which is one of the pathways that helps process estrogen. In theory, revving up that enzyme could lower your hormone levels a bit, and with birth control we always pay attention to anything that might weaken it.
That said, this is a minor, theoretical concern based only on test-tube research, not real people. Estrogen is mainly cleared by a different enzyme (CYP3A4), so the practical risk here is likely small. Still, since reliable birth control matters, mention the ashwagandha to your pharmacist or doctor before starting it, and watch for any breakthrough bleeding.

Your Ortho Evra patch releases two hormones, ethinyl estradiol and norelgestromin, that your liver clears partly through an enzyme called CYP3A4. In lab dish studies, ashwagandha seemed to speed up that enzyme. In theory, that could lower your hormone levels a little, which is the opposite of something blocking the enzyme.
Here's the honest part: this is only test-tube evidence, rated very weak, and the real-world effect is rated minor. We don't have human studies showing it actually causes patch failure. Still, since these hormones prevent pregnancy, even a small drop matters. If you want to use ashwagandha, chat with your pharmacist or doctor first, and consider a backup method if you have any concerns.
