Theoretically, taking ashwagandha with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that ashwagandha might lower systolic and diastolic blood pressure. Theoretically, ashwagandha might have additive effects when used with antihypertensive drugs and increase the risk of hypotension.

Here's the thing to keep in mind: riociguat (Adempas) works by relaxing and widening your blood vessels, which already lowers blood pressure. That's exactly how it helps your lungs, but it also means low blood pressure is one of its known side effects. Ashwagandha may have a mild blood-pressure-lowering effect of its own, so stacking the two together could, in theory, push your blood pressure down a bit further.

If that happens, you might feel dizzy, lightheaded, faint, or wobbly when you stand up. I'll be honest with you though, this concern comes mostly from animal studies, so it's more theoretical than proven. Still, since riociguat already affects blood pressure, please check with your pharmacist or doctor before adding ashwagandha rather than starting it on your own.

Here's the gist for Adempas (riociguat). Your body uses an enzyme called CYP3A4 to help break this medicine down. Some early lab studies suggest ashwagandha may speed that enzyme up a bit. In theory, that could clear riociguat from your system a little faster and make it work less well, meaning more shortness of breath, less stamina, or your pulmonary hypertension not being as controlled as it should be.

I want to be honest about how soft this evidence is. It's based only on test-tube research, not real people, and it's rated as a minor, possible concern. Still, since riociguat is a drug where steady levels really matter, it's worth a quick chat with your pharmacist or doctor before you add ashwagandha. Please don't change your dose on your own.

About Riociguat

What Is Riociguat Used For?

Brand name: Adempas

Riociguat tablets — sold under the brand names Adempas and Riociguat — are approved for adults with two types of high blood pressure in the lungs. The first is chronic thromboembolic pulmonary hypertension (CTEPH), a condition caused by blood clots that have blocked or narrowed the arteries in the lungs. Riociguat is used when surgery has not fully fixed the problem, or when surgery is not an option.

The second is pulmonary arterial hypertension (PAH), a progressive disease where the arteries in the lungs become stiff and narrowed. In PAH, riociguat helps improve exercise ability, functional class (how well you can get around day-to-day), and slows disease worsening. It can be used alone or combined with other PAH medicines called endothelin receptor antagonists or prostanoids.
