Ruxolitinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ashwagandha interacts with.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.

Here's the situation with these two. Your body relies on a liver enzyme called CYP3A4 to break down ruxolitinib (Jakafi or Opzelura). Some early lab research hints that ashwagandha may speed up that enzyme. If that happened in real life, your body could clear ruxolitinib a little faster, which in theory might make it work less well at controlling your myelofibrosis, polycythemia vera, or graft versus host disease.

I want to be honest about how thin this evidence is. It comes only from test tube and animal studies, the interaction is rated minor, and we don't have human data showing it actually matters. Still, ruxolitinib is a serious cancer medicine, so don't add ashwagandha on your own. Run it by your oncologist or pharmacist first so they can weigh in.

Ashwagandha is an Ayurvedic herb most often taken to help with stress, anxiety, and sleep, and some small studies suggest it may help, though the evidence is still limited. About Ruxolitinib

What Is Ruxolitinib Used For?

Brand names: Jakafi, Opzelura

Ruxolitinib is approved to treat several serious blood and immune conditions in adults, and some in patients as young as 12. It is used for myelofibrosis (a bone marrow disorder that disrupts normal blood cell production), polycythemia vera (a condition where the bone marrow makes too many red blood cells), and two forms of graft-versus-host disease (GVHD) — a complication that can occur after a stem cell transplant.

For polycythemia vera, it is specifically for patients who have not responded well to, or cannot tolerate, a medication called hydroxyurea. For chronic GVHD, it is used after at least one or two other treatments have failed.
