ANTIHYPERTENSIVE DRUGS

Sitaxsentan sodium falls under ANTIHYPERTENSIVE DRUGS — the group Ashwagandha interacts with.
Animal research suggests that ashwagandha might lower systolic and diastolic blood pressure. Theoretically, ashwagandha might have additive effects when used with antihypertensive drugs and increase the risk of hypotension.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Sitaxsentan is an endothelin receptor blocker that was used to treat pulmonary arterial hypertension, and part of what it does is relax blood vessels and lower pressure inside the lungs. Ashwagandha may have a mild blood-pressure-lowering effect of its own. So in theory, taking the two together could nudge your blood pressure a little lower than expected, which might leave you feeling dizzy, lightheaded, or tired, especially when you stand up quickly.
I want to be honest about how soft this evidence is: it comes from animal studies, not from people taking these two together, so it is more of a caution than a proven problem. Still, if you take sitaxsentan and are thinking about ashwagandha, please check with your pharmacist or doctor first rather than starting it on your own.

Ashwagandha has been linked to cases of acute hepatitis, liver failure, hepatic encephalopathy, autoimmune hepatitis, the need for liver transplantation, and death due to liver failure.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with this combination. Sitaxsentan (Thelin) is known for being hard on the liver, which is exactly why people taking it need regular liver blood tests. Ashwagandha, while usually well tolerated, has been tied in rare reports to serious liver problems, including hepatitis and even liver failure. When you stack two things that can each stress the liver, the worry is that the risk adds up.
To be clear, this is a theoretical caution based on how these two behave, not proven cases of them together. Still, watch for warning signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and call your doctor right away if they show up.
