Ashwagandha and Temsirolimus

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Ashwagandha interacts with whole categories of medication. Temsirolimus falls into 2 of those categories, so it can interact with Ashwagandha in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

IMMUNOSUPPRESSANTS

1 of 2

Why this applies to Temsirolimus

Temsirolimus falls under IMMUNOSUPPRESSANTS — the group Ashwagandha interacts with.

Theoretically, taking ashwagandha might decrease the effects of immunosuppressants.
Ashwagandha has demonstrated immunostimulant effects in humans. Animal research has shown that ashwagandha can attenuate the immunosuppression caused by cyclophosphamide.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thinking on combining ashwagandha with temsirolimus (Torisel). Ashwagandha is known to gently stimulate the immune system, and one of the jobs temsirolimus does is quiet down immune-related cell signaling to slow tumor growth. So in theory, the herb could work against what this drug is trying to do, possibly making it a bit less effective.

I want to be honest about how soft this evidence is. It comes from lab and animal studies, not from people taking these two together, so we don't really know if it matters in real life. Still, with a cancer therapy this important, I wouldn't add ashwagandha on your own. Please run it by your oncologist or pharmacist first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Temsirolimus also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the basic idea with Temsirolimus (Torisel) and ashwagandha. Your body uses an enzyme called CYP3A4 to help process this cancer medication. In laboratory studies, ashwagandha appeared to speed up that enzyme. If that happened in a person, it could clear Temsirolimus a little faster and possibly make it slightly less effective at slowing the kidney tumor it's prescribed for.

I want to be honest about the strength of this, though. This concern comes only from test-tube research, not from people, and it's rated a minor, possible interaction. Still, since Temsirolimus is a serious cancer treatment where keeping the right level matters, please don't add ashwagandha without first checking with your oncologist or pharmacist.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Temsirolimus page

“Temsirolimus (Torisel), an infusion for advanced kidney cancer, deserves genuine wariness around supplements. Most interactions on record involve herbs that alter CYP3A4, the liver enzyme that clears the drug, so a capsule can quietly raise its levels and side effects or thin out its anticancer effect.”
