Lomitapide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP3A4 substrates.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Here's the gist for Lomitapide (Juxtapid). Your body relies on an enzyme called CYP3A4 to clear this medication. Lab studies suggest the beta-carboline compounds in ayahuasca can block that same enzyme. If that blocking happened in people, lomitapide could build up to higher levels than intended.

Why that matters: lomitapide already carries a real risk of liver injury and stomach problems like nausea, diarrhea, and belly pain, and higher blood levels could make those worse. Importantly, this is theoretical and only seen in test-tube research, not in actual patients, so I don't want to alarm you. Still, ayahuasca isn't safe for self-use anyway. Please loop in your doctor or pharmacist before mixing anything with lomitapide.

About Lomitapide

What Is Lomitapide Used For?

Brand name: Juxtapid

Juxtapid (lomitapide) is used alongside a low-fat diet and other cholesterol-lowering treatments — including LDL apheresis (a procedure that filters LDL from the blood) where available — to lower LDL cholesterol, total cholesterol, apolipoprotein B, and non-HDL cholesterol in patients with homozygous familial hypercholesterolemia (HoFH). HoFH is a rare, inherited condition that causes extremely high LDL cholesterol levels from birth.

Juxtapid is approved for use in adults and in children aged 2 years and older with HoFH. It has not been proven safe or effective for other types of high cholesterol, including the more common heterozygous familial hypercholesterolemia (HeFH). Its effect on heart attacks, strokes, or other cardiovascular events has not been established.
