Bacopa + Acetaminophen, Chlorpheniramine, Dextromethorphan
Acetaminophen, Chlorpheniramine, Dextromethorphan brand name: Coricidin II Extra Strength Cold and Flu.
Acetaminophen, Chlorpheniramine, Dextromethorphan falls into 3 drug categories that Bacopa can interact with; the most serious is rated moderate. Theoretically, bacopa might increase the levels and adverse effects of CYP1A2 substrates.

These two interact in 3 distinct ways. Each one is explained separately below.
Why Are There 3 Interactions?
Bacopa interacts with whole categories of medication. Acetaminophen, Chlorpheniramine, Dextromethorphan falls into 3 of those categories, so it can interact with Bacopa in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
1 of 3Why this applies to Acetaminophen, Chlorpheniramine, Dextromethorphan
Acetaminophen, Chlorpheniramine, Dextromethorphan falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Bacopa interacts with.
Research on the effects of bacopa extracts on CYP1A2 enzymes is conflicting. Some in vitro evidence shows that bacopa extract can moderately and non-competitively inhibit CYP1A2, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This Coricidin product is a combo of acetaminophen (a pain and fever reducer), chlorpheniramine (an antihistamine), and dextromethorphan (a cough suppressant). The thinking here is that bacopa might slow down a liver enzyme called CYP1A2, which could let certain medicines build up a bit higher than usual. In theory, that could mean more of the typical side effects from this cold medicine, like drowsiness, dizziness, dry mouth, or a foggy, sleepy feeling.
I want to be honest with you though: this is only based on lab and test-tube studies, and even those results disagree with each other. None of these three ingredients lean heavily on CYP1A2 for clearance, so a real-world problem is unlikely. Still, if you tend to get very sleepy from this cold medicine, mention the bacopa to your pharmacist.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 3Acetaminophen, Chlorpheniramine, Dextromethorphan also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This cold and flu combo (Coricidin) packs three ingredients, and one of them, dextromethorphan, is partly cleared by a liver enzyme called CYP3A4. The thinking here is that bacopa might slow that enzyme down a little, which could let dextromethorphan build up. If that happened, you might feel more drowsy, dizzy, or foggy than usual.
I want to be honest about how shaky this is. The evidence comes only from lab dish studies, and even those disagree, with some showing little real effect. So for most people taking bacopa now and then, this is unlikely to cause trouble. Still, since both can make you sleepy, it's worth a quick check with your pharmacist or doctor before combining them regularly.

ANTICHOLINERGIC DRUGS
3 of 3Acetaminophen, Chlorpheniramine, Dextromethorphan also falls under ANTICHOLINERGIC DRUGS.
Bacopa seems to inhibit acetylcholinesterase and might increase acetylcholine levels, which could counteract the effects of anticholinergic drugs. Similarly, anticholinergic drugs might counteract the cholinergic effects of bacopa.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
The piece of this combo cold medicine that matters here is the chlorpheniramine, an older antihistamine that works by blocking acetylcholine in the body. Bacopa may do the opposite, gently raising acetylcholine levels. So in theory, the two could blunt each other a little. Bacopa might slightly soften the drowsiness and dry-mouth feel of the antihistamine, while the antihistamine might dull whatever memory benefit you hope to get from bacopa.
I want to be clear that this is a theoretical concern based on how each works, not something proven in people. For most folks taking an occasional dose, it is unlikely to cause a real problem. The acetaminophen and dextromethorphan parts are not involved. If you take bacopa daily, just mention it to your pharmacist or doctor.

From our pharmacist’s overview on the Acetaminophen, Chlorpheniramine, Dextromethorphan page
“Alcohol is the risk to understand first. It carries major ratings alongside the acetaminophen, chlorpheniramine, and dextromethorphan combination sold as Coricidin II Extra Strength Cold and Flu, because acetaminophen already taxes the liver, and heavy drinking on top of it raises the chance of genuine liver injury.”
