Voxelotor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP3A4 substrates.
Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

Here's the gist for Oxbryta (voxelotor). Your liver clears this medicine largely through an enzyme called CYP3A4. In lab dish studies, bacopa might slow that same enzyme down a bit. If it did, voxelotor could build up slightly and you might notice more of its usual side effects, like headache, nausea, stomach pain, or a fever or rash.

I want to be honest with you though: this is only a theoretical concern from test-tube research, and the studies actually disagree with each other. There's no strong proof it causes a real problem in people. Voxelotor is not a prodrug, so it doesn't get weaker here. Still, since this is an important sickle cell medicine, it's worth a quick chat with your pharmacist or doctor before adding bacopa.

Oxbryta (voxelotor) is approved to treat sickle cell disease (SCD) in adults and in children as young as 4 years old. It works by targeting the underlying problem in sickle cell disease — the abnormal hemoglobin that causes red blood cells to take on a rigid, sickle shape.

This approval was granted under the FDA's accelerated approval pathway, based on the drug's ability to raise hemoglobin levels. Continued approval may depend on results from confirmatory clinical trials showing broader clinical benefit.
