Lansoprazole, Amoxicillin, Clarithromycin falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Cannabidiol may increase levels of drugs metabolized by CYP2C19.
Research shows that cannabidiol inhibits CYP2C19. In clinical studies and case reports, cannabidiol use resulted in significant increases in the serum levels of topiramate, methadone, citalopram, omeprazole, and N-desmethylclobazam, the primary active metabolite of clobazam. These chemicals are metabolized by CYP2C19. Concomitant use of cannabidiol with CYP2C19 substrates may increase the risk for adverse effects from these substrates.

Prevpac is a combo pack used to treat H. pylori stomach infections. The piece that matters here is lansoprazole, the acid-reducing part, which your body partly clears using an enzyme called CYP2C19. CBD can slow that enzyme down. So when CBD and lansoprazole are taken together, lansoprazole levels can climb higher than expected.

The good news is that lansoprazole is generally well tolerated, so a modest bump usually isn't dangerous. You might just notice more side effects like headache, nausea, or diarrhea, and diarrhea can already be common with this combo because of the antibiotics. This is a fairly short course of treatment, so the window for any problem is small. Still, since the evidence for CBD blocking this enzyme is solid, it's worth a quick chat with your pharmacist before mixing them.

Theoretically, CYP3A4 inhibitors might increase cannabidiol levels.
Cannabidiol is a substrate of CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 enzymes might increase levels of cannabidiol, increasing its effects and adverse effects.

This combo pack (Prevpac) is used for about two weeks to treat an H. pylori stomach infection. The piece that matters here is clarithromycin, the antibiotic in the kit. Clarithromycin is a fairly strong blocker of an enzyme called CYP3A4, which is one of the main pathways your body uses to break down CBD.

So in theory, while you're on this short course, your CBD could clear more slowly and build up a bit. That might mean more of CBD's usual effects, like drowsiness, an upset stomach, diarrhea, or feeling more tired than normal. Keep in mind this is a theoretical concern based on how the drugs work, not something proven in studies, and it's only for the short time you take Prevpac. Still, it's worth a quick chat with your pharmacist before mixing the two.

In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Prevpac is a combo pack for treating H. pylori: lansoprazole (an acid reducer), amoxicillin (a penicillin antibiotic), and clarithromycin (a different antibiotic). The piece that matters here is clarithromycin, which is partly handled by the CYP3A4 enzyme. CBD can slow that enzyme down, so clarithromycin levels could drift a bit higher than usual.

In real terms, that might mean a touch more stomach upset, a metallic taste, or, more importantly, a slightly greater chance of heart rhythm changes (QT prolongation), since clarithromycin can already nudge that. None of these are guaranteed, and this is usually a short, defined course of treatment. Still, before adding CBD, check with your pharmacist or doctor so they can confirm it's fine for you. Don't stop your H. pylori therapy on your own.
