Bedaquiline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the gist: your body uses a liver enzyme called CYP3A4 to clear bedaquiline (Sirturo) out of your system. Lab studies hint that cascara sagrada might gently speed up that enzyme. If that happened, your body could clear bedaquiline a little faster, which in theory might lower its levels and make it slightly less effective against your MDR-TB.

Now, the important part: this is only based on test-tube research, and the effect was far weaker than known boosters like rifampin. So I wouldn't lose sleep over it. But TB treatment is too important to leave anything to chance, and cascara has its own issues with cramping and fluid loss. Please loop in your TB doctor or pharmacist before using cascara so they can keep your treatment on track.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
